Prenatal Yoga & Exercise Classes:
Prenatal yoga classes may entail physical activity. Such physical activity may be difficult and may cause or aggravate a physical injury or medical condition. The program may not match your fitness level and if you are concerned that you are not physically fit, please check with your physician before practicing.
Always consult with a physical prior to participating in this program to make sure that you are in good physical health. By following the instruction in this video you warrant that you are physically fit and have no medical condition or injury which would prevent your full participation in the program.
If you feel discomfort during this program, immediately stop and consult a physician.
Activities Involving Your Children:
You acknowledge that you understand the nature of the activities your child is participating in and that your child is in the proper physical condition and capable of participating in the related activities, understanding that Monica + Andy is not in any way responsible for making such a determination.
I authorize and agree that Monica + Andy may use photographs, videos, or my likeness of myself or my child as needed for its record-keeping, advertising, and/or public relations projects and that I have no rights to the same and will not be compensated for the same.
Monica + Andy and anyone involved in the creation of these classes are not responsible for any injury, condition or damages that you or your child may sustain as a result of following instruction from the class. By participating in this program you forever release, waive, discharge, and covenant not to sue Monica + Andy for any injury, conditions or damage you may experience. Please participate at your own risk.
In-Person Experiences and Classes:
I voluntarily seek to participate in events and activities provided by Monica + Andy.
I acknowledge the contagious nature of the Coronavirus/COVID-19 and that Monica + Andy has put in place preventative measures to reduce the spread of Coronavirus/COVID-19. I will follow all Centers for Disease Control and Prevention (CDC) guidance, and all posted instructions while attending Monica + Andy events and activities.
As a condition of my participation in this event, I, do hereby expressly waive any rights against and release and hold harmless Monica + Andy from and against any and all claims, suits, demands, losses, damages, expenses, or liability that may arise during or as a result of my presence and my child/children’s presence at the premises, including but not limited to any such liability related to or arising out of illness, injury, or associated with the Coronavirus/COVID-19.
I understand, and it is my express intent, that this release and waiver of all liability releases Monica + Andy from such liability. In the event that I or my family member is injured, becomes ill, or suffers complications due to Coronavirus/COVID-19, allegedly as a result of my participation in a Monica + Andy event or activity. I agree to release and hold harmless Monica + Andy in the same manner and to the same extent as set forth above.
BY COMPLETING THE REGISTRATION FOR THIS EVENT, I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS RELEASE AND FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS CONCERNING LIABILITY AS DESCRIBED ABOVE.
